APPLICATIONS AND FORMS
Prior to Construction of a new Public Water Well and Public Water System, a Preliminary Technical Report is required to be submitted to the State Water Resources Control Board District Office and Yuba County Environmental Health.
A Public Water Program Domestic Water Supply permit is required in order to operate a new Public Water System. The application form along with the appropriate Bacteriological Sample Siting Plan (BSSP), Emergency Notification Plan, Cross Connection Control Plan, and TMF Assessment must be completed and submitted to the LPA prior to operation.
A Domestic Water Supply permit amendment is required if changes or modifications are going to be made to the existing permitted public water system. Changes and modifications include, but are not limited to the following:
- Adding a new storage tank
- Adding a new source
- Changing the status of an existing source (e.g., from standby to active)
- Adding or changing the treatment plant
- Changes in classification of the Public Water System
- Preliminary Technical Re Guidance Document
- Domestic Water Supply Permit Application
- Domestic Water Supply Permit Amendment Application
- TMF Assessment
- Bacteriological Sample Siting Plan (BSSP)
- Emergency Notification Plan (ENP)
- Cross Connection Control Plan
- Chlorination Log
